The Ontario government is advancing its Primary Care Action Plan with the launch of a provincewide Primary Care Medical Record system. It aimed at integrating patient records, reducing paperwork for doctors, and improving patient care. The system will provide a secure, interoperable platform that allows clinicians to access patients’ medical histories, test results, and medications across the province, reducing delays and costly repeat testing.

“By launching this system, we are making care more connected and convenient, so patients don’t have to repeat their medical history and clinicians can focus on care,” said Sylvia Jones, Deputy Premier and Minister of Health.

The initiative comes as Ontario surpasses its 2025-26 goal of connecting 300,000 patients to a primary care provider, reaching 330,000 as of January 2026. Additionally, 124 new or expanded primary care teams will soon be established, expected to connect another 500,000 Ontarians to care.

While 90% of Ontario family physicians currently use electronic medical records, the fragmented system has limited sharing between providers. The new system will ensure a complete, secure patient record accessible across the province, potentially saving patients hundreds of dollars in transfer fees.

The government plans to support physicians transitioning to the new system and will conduct an open competitive procurement process to establish a Vendor of Record. The initiative is also part of broader efforts to strengthen Ontario’s health workforce and ensure timely, high-quality care for residents, including injured workers.

Ontario has allocated $3.4 billion from 2025 to 2029 to expand primary care under the action plan, reinforcing the province’s commitment to a modern, connected health-care system.
